Records Requests

Records Requests

Request a 911 recording, transcript or your own records. Pick the right form below.

How It Works

Before you request

A few things apply to most requests, no matter which form you use.

  • A small fee or refundable deposit may apply, depending on the request
  • Government-issued photo ID is required to pick up records in person
  • Email and fax are not secure; choose mail or in-person pickup for sensitive records
  • Records not picked up within 30 days are destroyed, with no refund
